Annual Reviews
Good management of people with long-term conditions has been a key priority of the NHS since the early 1990s. At that time it was recognised that if people with long-term conditions were managed effectively in the community, they should remain relatively stable and enjoy a quality of life free from frequent crises or observed increases in hospital visits.
Long-term condition management covers the following conditions:
- Learning difficulties
- Mental health
- Diabetes
- Dementia
- COPD
- Asthma
- Hypertension
- Ischaemic heart disease
- Epilepsy
- Hypothyroidism
- Chronic kidney disease
We try to ensure that all of your annual reviews are done together within your birthday month to achieve the following:
- Reduction in the number of appointments each patient requires.
- Allow patients to take more control of when they attend for their reviews as they will know in advance when their review is due, and can better book appointments to fit in with their own commitments.
- A more focus approach to patient care.
