Care Navigator Team

Our team consists of our Assistant Practice Manager & Senior Care Navigator Rhian Hughes supported by 6 Care Navigators. They are here to help you with appointments and any questions you may have concerning the results of tests, hospital referrals etc. They all regularly attend relevant training courses.

What is a Care Navigator?

A Care Navigator will make sure you get help from the right person at the right time. To do this, Care Navigators will need to ask why you are calling so that they can arrange the right care or support for you. Care navigators have been trained to keep information  confidential  and to provide a safe service.
